What to Expect

Your provider will review your menstrual history, perform a pelvic exam, and may order blood tests to check hormone levels and rule out thyroid or clotting disorders. An ultrasound may be performed to evaluate the uterus for fibroids, polyps, or other structural causes. Treatment depends on the cause and may include hormonal medications, an IUD, endometrial ablation, or surgical intervention.

What counts as abnormal bleeding?
Bleeding is considered abnormal if periods last more than 7 days, occur more often than every 21 days, require changing protection every 1-2 hours, include large clots, or occur after menopause.
What causes abnormal bleeding?
Common causes include hormonal imbalances, fibroids, polyps, PCOS, thyroid disorders, and sometimes precancerous or cancerous conditions. Your provider will determine the cause.
Should I be worried?
Most causes of abnormal bleeding are treatable and not serious. However, it's important to be evaluated to rule out concerning conditions and get relief from symptoms.
What is endometrial ablation?
Endometrial ablation is a minimally invasive procedure that destroys the uterine lining to reduce or stop heavy bleeding. It's an option for women who don't plan future pregnancies.
What are uterine polyps and how are they treated?
Uterine polyps are small, usually benign growths of the uterine lining that can cause heavy periods, bleeding between periods, spotting after sex, or bleeding after menopause. They are common, especially in the 40s and 50s, and most are not cancerous — but postmenopausal bleeding from a polyp always deserves evaluation. We typically find them with a pelvic ultrasound or a saline-infusion sonohysterography (a quick in-office scan that gently distends the uterus with sterile saline so polyps stand out clearly). The standard treatment is hysteroscopic polypectomy: a thin scope is passed through the cervix and the polyp is removed in a short outpatient procedure, usually with no incisions and a same-day return home.
